ABSTRACT

Objective:To evaluate the effect of electroacupuncture (EA) on pyroptosis in renal tubular epithelial cells of rats with acute kidney injury (AKI) induced by endotoxin.Methods:Twenty-four healthy clean-grade Sprague-Dawley rats of either gender, aged 6-8 weeks, weighing 160-182 g, were divided into 4 groups ( n=6 each) using a random number table method: control group (group C), group AKI, EA plus AKI group (group EA), sham EA at non-acupoint plus AKI group (group SEA). The model of endotoxemia was established by intraperitoneally injecting 10 mg/kg lipopolysaccharide.Bilateral 30 min EA stimulation of Zusanli and Shenyu (according to atlas of animal acupoint) was performed starting from 5 days before establishing the model (once a day) and at 30 min before lipopolysaccharide administration on the day of establishing the model, with disperse-dense waves, frequency of 15 Hz, and the needle was kept until 6 h after injection of LPS in group EA.EA was performed at the points 0.5 cm lateral to the acupoints of Zusanli and Shenyu in group SEA.At 6 h after LPS injection, blood was taken from the heart, and the concentrations of serum blood urea nitrogen (BUN) and creatinine (Cr) were detected by an automatic biochemical analyzer, and the serum concentrations of neutrophil gelatinase-associated lipocalin (NGAL), kidney injury molecule-1 (KIM-1), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) by enzyme-linked immunosorbent assay. The rats were then sacrificed, and the left renal cortex was obtained for determination of pyroptosis rate of renal tubular epithelial cells (by TUNEL). The right renal cortex was obtained to detect the expression of caspase-1 and IL-1β by Western blot, and the expression of caspase-1 mRNA and IL-1β mRNA was detected by real-time polymerase chain reaction. Results:Compared with group C, the concentrations of BUN, Cr, NGAL, KIM-1, TNF-α, and IL-6 were significantly increased, the pyroptosis rate of renal tubular epithelial cells was increased, the expression of caspase-1 and IL-1β protein and mRNA in the renal cortex was up-regulated in group AKI ( P<0.05). Compared with group AKI, the concentrations of BUN, Cr, NGAL, KIM-1, TNF-α, and IL-6 were significantly decreased, the pyroptosis rate of renal tubular epithelial cells was decreased, the expression of caspase-1 and IL-1β protein and mRNA in the renal cortex was down-regulated in group SEA ( P>0.05). Conclusion:The mechanism by which EA reduces AKI may be related to inhibiting pyroptosis in renal tubular epithelial cells of rats.

ABSTRACT

Objective@#To establish a prediction model for infectious disease index(IDI)by autoregressive integrated moving average(ARIMA),and to provide forcast of infectious diseases to the public. @*Methods@#The data of the percentage of influenza-like illness(ILI),the incidence rates of hand-foot-mouth disease(HFMD)and other infectious diarrhea(OID)from the 1st week of 2014 to the 14th week of 2018,and Breteau index(BI)from the 1st week of 2016 to the 14th week of 2018 were collected. ARIMA models were built to predict the risk indicators of ILI,HFMD,OID and BI. The weights of the four indicators were evaluated seasonally by the entropy weight method. Then the IDI was calculated and the data of ILI,HFMD, OID and BI from 15th to 19th week in 2018 was used for verification. @*Results@#The forecast was in summer,so IDI=ROUND(0.33×risk index of ILI percentage +0.47×risk index of HFMD incidence +0.10×risk index of OID incidence+0.10×risk index of BI). The predicted IDI would be 2(less safe)in the whole city and Xiangzhou District, and 1(safe)in Doumen District and Jinwan District. The consistency rates of IDI prediction was 97.50%,95.00%,97.50%,85.00% and 77.50% from 15th to 19th week in 2018,respectively.@*Conclusion@#It was feasible to use IDI for short-term risk prediction of infectious diseases.

ABSTRACT

Objective To evaluate the effect of sufentanil on apoptosis in spinal cord neurons of mice with peripheral nerve injury. Methods One hundred and fifty clean-grade healthy male BALB∕c mice, aged 6-8 weeks, weighing 18-22 g, were divided into 3 groups ( n=50 each) using a random number table method: sham operation group (group Sham), peripheral nerve injury group (group PNI) and sufentanil group ( group SF) . The model of unilateral sciatic nerve injury was established in PNI and SF groups. After establishing the model, sufentanil 5. 0 μg∕kg was intraperitoneally injected once a day for 3 consecutive days in group SF, while the equal volume of normal saline was given instead of sufentanil in Sham and PNI groups. Five mice in each group were sacrificed at days 1, 3, 7, 14 and 28 after surgery ( T0-4 ) , and L4-6 segments of the injure ipsilateral spinal cord were removed for examination of pathological changes ( with a light microscope) and for determination of neuronal apoptosis ( by TUNEL assay) . The ap-optosis index ( AI) was calculated. Five mice in each group were sacrificed at T0-4 , and L4-6 segments of the injured ipsilateral spinal cord were removed for detection of the expression of Bcl-2, Bax and cleaved caspase-3 by Western blot. The ratio of Bcl-2 expression to Bax expression ( Bcl-2∕Bax ratio) was calculat-ed. Results Compared with group Sham, the AI was significantly increased, the expression of Bcl-2 pro-tein was down-regulated, and the expression of cleaved caspase-3 and Bax was up-regulated in PNI and SF groups ( P<0. 05) . Compared with group PNI, the AI was significantly decreased, the expression of Bcl-2 protein was up-regulated, the expression of cleaved caspase-3 and Bax was down-regulated, the Bcl-2∕Bax ratio was increased (P<0. 05), and the pathological changes were significantly attenuated in group SF. Conclusion Sufentanil can inhibit apoptosis in spinal cord neurons of mice with peripheral nerve injury.

ABSTRACT

Objective To evaluate the effect of sufentanil on the expression of basic fibroblast growth factor(bFGF)in nerve tissues after peripheral nerve injury in mice. Methods One hundred pathogen-free healthy male Balb∕c mice, aged 6-8 weeks, weighing 18-22 g, were divided into 4 groups (n=25 each)using a random number table: peripherial nerve injury group(group PNI), high dose sufentanil group(group H), medium dose sufentanil group(group M)and low dose sufentanil group (group L). The model of unilateral sciatic nerve transaction was established in ketamine-anesthetized mice. Sufentanil 100, 50 and 25 μg∕kg were intraperitoneally injected immediately after establishment of the model once a day for 3 consecutive days in H, M and L groups, respectively. The equal volume of normal saline was given instead in group PNI. On 1, 2, 4, 8 and 12 weeks after establishment of the model, 5 mice were sacrificed and the nerve tissues were obtained from the site 05 cm up and down the lesion site of the nerve for examination of the shape of the myelin sheath of nerve fibers(with an electronic microscope). The expression of bFGF in the sciatic nerve tissue was detected by Western blot. Results The shape of medulla sheath was irregular, the thickness of myelin lamellae was thin, the separation of myelin lamellae was aggravate, demyelinate was found, and the proliferation of Schwann cells was not marked in group PNI. The shape of medulla sheath was regular, the thickness of myelin lamellae was dense, and the proliferation of Schwann cells was marked in group H. The shape of medulla sheath was irregular, the separation of mye-lin lamellae was observed, demyelinate was found, and the proliferation of Schwann cells was not marked in group L. Compared with group PNI, the expression of bFGF was significantly up-regulated in H, M and L groups(P<005). Compared with group L, the expression of bFGF was significantly up-regulated in H and M groups(P<005). Compared with group M, the expression of bFGF was significantly up-regulated in group H(P<005). ABSTRACT

Objective To discuss the application value, safety and feasibility of indwelling guidewire technique in performing mechanical thrombectomy for acute cerebral artery occlusion. Methods The clinical data of 15 patients with acute ischemic stroke, who were treated with mechanical thromnectomy at authors' hospital during the period from December 2015 to February 2016,were retrospectively analyzed. The diseases included middle cerebral artery occlusion (n=6),internal carotid artery and middle cerebral artery occlusion (n=5) and vertebral basilar artery occlusion (n=4).Indwelling guide-wire technique was adopted to quickly and accurately determine the vascular occlusion characteristics, then, endovascular mechanical thrombectomy was carried out. Results By using indwelling guide-wire technique, the occlusive features of the diseased arteries were successfully and precisely determined.Immediate recanalization of the occluded artery was obtained in 13 patients (87%).The blood flow classification score after thrombolysis in cerebral infarction (TICI) reached 3 points in 10 patients and 2b points in 5 patients. In 2 patients, the vascular recanalization procedure failed because the thrombus load was large, the length of occluded segment was long, and the effect of mechanical thromnectomy was poor.According to American National Institutes of Health Stroke Scale (NIHSS),the evaluation score was improved from preoperative (19.2±7.0) to postoperative (6.3±3.6),the difference was statistically significant (P<0.05).Three months after the treatment, the score measured by modified Rankin scale(mRS) was ≤2 points in 10 patients. Conclusion Mechanical thrombectomy is safe and effective for the treatment of acute cerebral artery occlusion. The indwelling guide-wire technique can safely, conveniently, quickly and accurately determine the characteristics of the occluded blood vessels, which is very helpful in assisting surgical manipulation, reducing procedure-related complications, and improving vascular recanalization rate.(J Intervent Radiol,2017,26:202-205)

ABSTRACT

Objective To explore the feasibility and safety of using 6 F guiding catheter to perform the carotid artery stenting (CAS) via transradial approach (TRA). Methods A retrospective analysis of 28 cases with carotid atherosclerotic stenosis who were using 6 F guiding catheter to perform the CAS via TRA, and the preoperative assessment confirmed that some of whom were difficult to operate via femoral artery, or couldn't tolerate the lying in bed after the operation, and the others were failure to perform via femoral artery. The atherosclerotic carotid stenosis lesion of 9 cases were located in the left internal carotid artery (ICA), and the other 19 cases were in the right ICA. All the patients were performed via the right radial artery approach, a 6 F guiding catheter was used to super-select the target vessel, implanted the distal protection device, then underwent balloon angioplasty and stent implantation. We observed and recorded that whether the guiding catheters were in the right places, the success rate of stent implantation, the incidence of radial artery spasm and the puncture site bleeding. The postoperative pulse and occlusion of radial artery, and the occurrence of cardio-cerebrovascular complications during perioperative period were also observed. The fluoroscopy time of surgery ray were also recorded, and compared with corresponding 30 cases who were performed CAS via the tranfemoral approach (TFA) by t test. Results The guiding catheters were in the right places and stenting success of all the 28 cases. There was no significant difference in the fluoroscopy time between TRA and TFA group [(8.6 ± 1.4) min vs. (9.0 ± 2.1) min,t=-0.717,P=0.477)]. Two cases appeared radial artery spasm after puncture, and 1 case experienced puncture site bleeding at 8 h after the operation. After examined the radial artery by color doppler ultrasound at 1 week after the operation, we found that blood flow of 27 cases were patency, but another 1 was slowed down, which was restored at 3 months follow up. None of the cases occurred cerebrovascular events in the present research. Conclusion Transradial approach for CAS using 6 F guiding catheter is safe and technically feasible.

ABSTRACT

Objective To explore the technical skills and short-term efficacy of Pipeline Embolization Device (PED) deployment for endovascular treatment of complex intracranial carotid aneurysm. Methods The medical charts of 6 cases who underwent PED implant procedure in our institute from Jan. to Jun. 2015 were reviewed, including 4 female and 2 male; the mean age was (56 ± 13) years, ranging from 34—70 years. All the 6 cases suffered from intracranial carotid aneurysm and PED implant procedures were performed to cover the aneurismal neck under general anesthesia, while dual antiplatelet treatment (Bayaspirin+Plavix) was administered. The basic characteristics and peri-procedural adverse events were recorded, interview and mRS score were done by telephone 1 month after procedure, clinical examination and DSA angiographic re-check were made 3 months following procedure, respectively. Status of aneurysm occlusion was evaluated by Raymond grading scale. Results All 6 aneurysms involved were unruptured. Technical success was achieved in all procedures for 6 cases, of which 3 cases were single PED implantation and the other 3 PED adjuncted to coiling. No procedure related complication was detected during 30 days after operation. Follow up with 1.5—4.8 months (median 2.8 months) were conducted and last follow up revealed good outcome (mRS<2) in all cases. The control angiography showed contrast filling aneurismal sac even after PED implantation, DSA recheck performed in 2 cases at 3 months after operation and revealed good obstruction of aneurysm (Raymond grade≤2). Conclusion Domestic preliminary study shows PED implantation seems to be a safe and effective modality for treatment of complex intracranial carotid aneurysms.

ABSTRACT

Objective To compare the perioperative complications of prophylactic use of three antiplatelet strategies in unruptured intracranial aneurysms treated by stent assisted coiling. Methods A total of 203 consecutive patients were brought into this retrospective study including the following three groups:the loading group (n=54), with a loading dose of 300 mg to 600 mg clopidogrel at 2 h to 24h before the stenting; tirofiban group (n=50), a loading dosage of tirofiban (8 μg/kg/min over 3 min) followed by a 0.1μg/kg/min maintenance dosage; dual antiplatelet group (n=99), dual oral antiplatelet drugs (clopidogrel 75 mg + aspirin 100 mg) pretreated for 3-5 days before the operation. Events of hemorrhage and thromboembolism were recorded and the complications were compared to assess the safety and efficacy of various antiplatelet strategies. Chi-square or Fisher exact tests were used for categorical variables. Results The hemorrhagic rates were 11.1% in loading group, 2.0% in dual antiplatelet group, and 0% in tirofiban group, respectively, while the thromboembolic rates were 7.4% in loading group, 4.0% in dual antiplatelet group, and 0% in tirofiban group, respectively. For total complications, significant difference (P0.05). The hemorrhagic rate in loading group was significantly higher than that of the dual group (P=0.023) and tirofiban group (P=0.027), while there was no significant difference between tirofiban group and dual antiplatelet group (P>0.05). In subgroup analysis of the loading group, the postoperative thromboembolic rate was significantly higher in those exposed to low molecular weight heparin than those not (P=0.039) with no increase of hemorrhagic events (P>0.05). Conclusions When compared with the dual antiplatelet strategy, tirofiban strategy may be used as a new prophylactic protocol in unruptured intracranial aneurysms treated by stent assisted coiling. Those treated by low molecular weight heparin postoperatively after receiving dual antiplatelet therapy may increase the hemorrhagic risk, although there was a significant decrease in thromboembolic events postoperatively.

ABSTRACT

Objective To construct canine bifurcation aneurysms suitable for evaluating the exploration of endovascular devices for interventional therapy by endovascular technique. Methods The right common carotid artery of six dogs was expanded with a pliable balloon by means of endovascular technique, then embolization with detached balloon was taken at their originations DAS examination were performed on 1,2,3 d after the procedurse. Results 6 aneurysm models were created in six dogs successfully with the mean width and height of the aneurysms decreasing in 3 days. Conclusions This canine aneurysm model presents the virture in the size and shape of human cerebral bifurcation saccular aneurysms on DSA image, suitable for developing the exploration of endovascular devices for aneurismal therapy. The procedure is quick, reliable and reproducible.

ABSTRACT

AIM:To study whether three kinds of rabbit corneal cells can grow well on fibrin glue (FG) constructed in vitro, and investigate the feasibility of FG for the scaffold of cell sheet engineering. METHODS: Three kinds of corneal cells were seeded on FG which was produced in vivo. Cell growth on FG was examined as follows: (1) by inverted microscope; (2) histologically by hematoxylin and eosin; (3) by scanning electron microscopy. RESULTS: Fibrin glue prepared was smooth and transparent. With cell growth, FG degradated partly, then obtained cell sheet engineering only with a small amount of FG. Corneal cells generated well on the fibrin glue in vitro and maintained the physiological state of cells. Corneal epithelial cells formed unilaminar and stratified layers and cellular joins. Corneal endothelial cells formed round or polygon, the same size cells and lined up tightly. Corneal stroma cells formed triangle and arborization, cell-cell junction was obvious, and formed network link. CONCLUSION: Fibrin glue is well compatible with three kinds of corneal cells, which can construct tissue engineered cell sheet with fibrin glue, so as to reconstruct ocular surface.
